This initiative strives to meet three goals: that children are born healthy, that children live in safe and nurturing environments, and that children enter school ready to learn. Through financial support with home visiting to improve birth outcomes, to collaborative membership of community and service organizations, efforts and events are held that address these goals. The collaborative meets monthly and anyone interested in furthering the goals of Success By 6 are invited to attend. For more information about Success By 6 call Imani Williams-El at 410-675-2125. Program Details Healthy Relationship InitiativeProvide BCHS pregnant and post-partum clients and their significant others with tools to maintain a healthy relationship and marriage, if they decide to wed. Program Details In Home/In Center Family Planning ProgramSince 2003, interconception care services have been provided to postpartum clients by a Certified Registered Nurse Practitioner (CRNP), who is fully integrated into Healthy Start case management operations. Among postpartum women who have had a miscarriage or a fetal or infant loss, and would otherwise not be eligible for Healthy Start services, the Interconception Care Initiative provides a framework of services into which these women are recruited and enrolled. The Nurse practitioner provides postpartum clinical examinations in the home or at the Healthy Start site to ensure compliance with postpartum clinical care. In addition, the CRNP is available to provide a myriad of other clinical and educational services to all clients. The CRNP visits clients quarterly from 3-24 months postpartum to provide continued family planning education and counseling services including prescribing and administering contraceptive methods.
